Make sure your site signup page is called "signup.php", make sure you don't use cloudflare or similar. Re-check your code just in case. But basically, look for a similarity inside the signup.php code, and replace the old one with the new one. If you cant find it, look for a word near the object you want to change, and look for that word in the signup.php, after that the code you need to replace will be near that word.
